Large cavitary pulmonary infarctions are extremely rare. Here, we report an unusual case of large pulmonary infarction due to pulmonary embolism (PE). This was complicated by secondary infection with Actinomyces leading to cavitary abscess formation. The patient had minimal non-specific symptoms despite extensive involvement and was treated with lobectomy, antibiotics, and anticoagulation, with expedited recovery.
